SUMMARY OF TERMS AND ACCOUNT CHARGES

As a recurring dues member, you are agreeing to pay for a monthly reoccurring charge to participate in services and activities at Kandara, LLC.

        - Your recurring dues billing will begin on the first day of payment, and will continue on the same day each month thereafter until you properly cancel per the terms of this Agreement.

        - This is a 3 month term agreement which then moves to a reoccurring month to month membership until canceled with a 30 day written notice provided to transform@kandarastudio.com.

        - By agreeing to the contract, you expressly authorize Kandara, LLC or its third-party billing services provider, to draft your account each month for your agreed upon dues, fees or other charges.

TERMS OF YOUR RECURRING DUES MEMBERSHIP

  Membership is to begin on studio opening day (anticipated 10/8/26) or Day of purchase if occurring after opening

       Monthly Dues: $111.00 + tax

       Recurring Dues: Period 30 days

       Minimum Requirement of 3 month membership. Membership continues until notice of written cancellation is provided to transform@kandarastudio.com

CONSENT TO AUTO-RENEWAL

 (All references to “I” or “me” in this section shall refer to you)

UNLIMITED MEMBER: I understand that my membership will continue to renew monthly until I properly cancel, that proper cancellation requires me to provide Kandara, LLC with advance written notice of cancellation at least 30 days prior to scheduled billing via email to transform@kandarastudio.com with the subject line "Membership Cancellation." If confirmation of cancellation is not obtained via written response within 48 hours of contact, please reach out again.

AUTHORIZATION FOR PREAUTHORIZED PAYMENTS: (All references to “I” or “me” in this section shall refer to you) By signing below, I am authorizing to initiate transfers, whether by EFT from the designated bank account tied to this Agreement for purposes of paying, on a recurring basis, all or a portion of the dues, fees and charges which I owe under the Agreement. I understand and agree that: (a) dues, fees and charges include, but may not be limited to, enrollment fees, membership dues, service charges, late fees, applicable taxes, and/or fees for uncollected monthly dues; (b) Kandara LLC may transfer funds from my designated account for any retail transactions or online purchases initiated by me; (c) dues, fees and charges will be drawn on or about the dates set forth in the Payment Schedule; (d) this preauthorization will remain in effect until all of my payment obligations under the Agreement have been satisfied; and (e) debited amounts may vary each month based on additional amounts which I may owe under this Agreement.

CLASS CANCELLATION

 All classes that are reserved should be canceled four (4) hours prior to the class beginning. If you must cancel a class within four (4) hours of the class starting and you have an Unlimited Package there will be a $10 late cancel fee charged to your payment on file. If you do not show up for a class that was previously scheduled, there is a $10 fee that is charged to your payment on file.

 If you are on a Class Package Membership, you will lose the class credit if you cancel within four (4) hours of the class or do not show up for the previously scheduled class.

NOTICE: WV STATE LAW REQUIRES THAT WE INFORM YOU THAT SHOULD YOU (THE BUYER) CHOOSE TO PAY FOR ANY PART OF THIS AGREEMENT IN ADVANCE, BE AWARE THAT YOU ARE PAYING FOR FUTURE SERVICES AND MAY BE RISKING LOSS OF YOUR MONEY IN THE EVENT THIS STUDIO CEASES TO CONDUCT BUSINESS. FITNESS STUDIOS DO NOT POST A BOND, AND NO OTHER PROTECTIONS MAY BE PROVIDED TO YOU SHOULD YOU CHOOSE TO PAY IN ADVANCE
